In Albania, many vital services financed with taxpayers' money are in the hands of private entities, which, taking advantage of high-level deals, treat them as monopolies. Such is the service for kidney patients who are at the last stage of the functioning of these organs and undergo mandatory hemodialysis. For years, this service has benefited from a dubious clientelistic scheme "American Hospital", which in recent years to dispel the noise that has been made in various media regarding the problem of this monopoly. In order to make a game about the documents, in order to be formally in order with the state, "American Hospital" invented another shareholder to whom it has given a part of this service, but behind which the same shareholders stand with the "American Hospital". Recently, this thing has become the object of complaints and dissatisfaction from patients who constantly receive this service, as well as it has been casually taken under "investigation" by some state institutions. But it seems that the shareholders of the "American Hospital" know how to "solve" any kind of problem when they are put into play or blocked by some state institution. This is also thanks to the support they constantly find from high institutions, ministers or deputies.

Fat bills

In the 2023 and 2022 reports of KLSH, it is clearly established how the concessions continue to reap big bills, with those in the Health sector at the top. Even the concessions that hospital services have are being collected without criteria from the state budget due to mismanagement. This is how the KLSH reports in the audit carried out in this institution. Office data showed that in 2022, funds for the hospital service reached 29.4 billion ALL, but the amount of money that went to finance the three PPP contracts amounted to 4.1 billion ALL or 14 percent of the hospital budget. "The cost of PPP contracts significantly exceeded the value of primary hospital services. For example, data show that the cost of drugs for hospitals was 3.6 billion ALL or about 13 percent lower than PPP funds in the hospital service. Three PPP contracts are in operation in the hospital service, that of sterilization, dialysis and laboratory. In 2022, contracts for the sterilization of surgical equipment were financed with ALL 1.7 billion, Dialysis with ALL 835 million and the Laboratory with ALL 1.6 billion. For the two PPPs of the laboratory and sterilization, the value of the financing has exceeded the initial forecasts, as the High State Control has discovered many abuses during their implementation", the report states. Funds for hospital services increased in recent years after the pandemic, but the weight of concessions has grown faster. However, this money has gone into the hands of the concessionaires and there is no free healthcare for the citizens.

How has the American Hospital stolen all this time Albanian taxpayers' money?

Kontrolli i Lartë i Shteti ka gjetur një mal me shkelje për ofrimin e dializës në vend. Janë të shumtë ata që kanë probleme me veshkat dhe për fatin e tyre të keq duhet të bëjnë dializë. Qeveria vendosi që këtë shërbim ta ushtrojë privati. Koncesioni  u dha në vitin 2015 te kompania “Dia Vita”, pas së cilës fshihet Spitali Amerikan. Por KLSH në një auditimin që ka bërë, ka zbuluar se deri një vit më parë, ndaj koncesionarit nuk është bërë asnjë kontroll se si e ka zbatuar kontratën. “Nuk ka patur monitorim të vazhdueshëm nga institucionet përgjegjëse (MSHMS dhe FSDKSH) për zbatimin e kontratës së koncesionit dhe kontratave të tjera të lidhura ndërmjet institucioneve në varësi të FSDKSH për ofrimin e shërbimit të Hemodializës. Sipas programit kombëtarë mbi parandalimin, zbulimin e hershëm dhe mjekimin e sëmundjeve kronike të veshkave në Shqipëri, i hartuar nga qendra spitalore universitare “Nënë Tereza”, shërbimi, nefrologji-dialize-transplant, duhet të kryet monitorim vjetorë për protokollin e ndjekur të hemodializës, FSDKSH duhet të ketë në përbërje një mjek nefrolog në grupet e kontrollit. Faktikisht monitorimi që kryen FSDKSH nëpërmjet drejtorive rajonale për qendrat e HD bëhet vetëm për elementet financiare të numrit të seancave. Gjithashtu edhe MSHMS nuk e ka monitoruar në mënyrë periodike sipas kërkesave të kontratës koncensionare shërbimin e kryer nëpër spitalet publike", thekson KLSH. Sipas raportit, MSHMS në rolin e autoritetit kontraktor, monitorimin e shërbimit të dializës të ofruar nëpërmjet PPP e ka filluar në vitin 2022, pra rreth 7 vite nga fillimi i ofrimit të shërbimit nga koncensionari. Po ashtu KLSH ka konstatuar se kemi të njëjtin skandal si me Check Up, për këtë shërbim që e ofron privati, përdoren mjekët e spitaleve publike. “Grupi i auditimit gjatë fazës së terrenit u shoqërua nga DRF në disa vizita në qendrat e dializës në Elbasan, Berat, Fier, Lezhë, Shkodër, Korçë, Vlorë ku konstatoi se përgjithësisht, mjekët nefrolog të cilët mbulonin shërbimin për qendrat e dializës ishin të njëjtët që shërbenin në spitalet publike. MSHMS nuk ju përgjigj pyetjes se “Cilat kanë qenë arsyet që nuk është ngritur grup pune dhe nuk është monitoruar periodikisht shërbimi i hemodializës i ofruar nëpërmjet kontratës koncesionare?””, thekson KLSH. Skandali tjetër është se Spitali Amerikan nuk ka investuar në pajisje dhe infrastrukturë. Sipas KLSH, kontrata shprehet se ai duhet të investojë dhe në fund të marrëveshjes pajisjet dhe gjithë investimi i kalon shtetit. “Sipas kontratës së koncensionit, ri investimit në pajisje mjekësore do të behet 100% dhe do të fillojë në 4 vitet e fundit. Koncensionari nuk ka filluar të ri investojë në pajisje mjekësore edhe pse kontrata e koncesionit është futur në vitin e tetë të zbatimit. Pajisjet mjekësore, mbas përfundimit të kontratës do mbeten prone e spitaleve publike”, thekson KLSH. Tjetër konstatim është se nuk bëhet identifikimi i disa pacientëve kronik të cilët janë projektuar që shërbimin do ta kryejnë pranë qendrës së dializës së koncensionarit, por kanë zgjedhur që shërbimin ta kryejnë pranë qendrave të tjera të dializës që mbulohen nga spitali jopublik, ç’ ka krijon premisa për pagesë të dyfishtë të një seance dialize.
